WELCOME TO THE FORM TO SUBMIT YOUR SELF-CLASSIFICATION
If you have impairments in all 4 tested categories (Upper extremities, Trunk, Hips and Coordination), the classification process is fairly long – about 2 hours – and takes you through 17 different tests. We advise you to take your time and have breaks if needed. Getting the help of a physical therapist is highly recommended!

We also invite you to visit the classification page. There, you can find a spreadsheet you can use to to enter your numbers as you measure them. Doing so will directly give you your Functional Scores for 2024. You can also print a PDF form if you prefer pen and paper. If you use this form to send us your result, we will calculate your Functional Scores for you and send them to you by email.
